You take the diff out of the housing and make a new housing which locates the dif by the shear pins normally used for locating the ring gear. Then you fix that housing in such way that it can rotate freely on bearings. Then you bolt a sprocket to the housing and call it a day.
Taylor Race is one such manufacturer of differentials for open wheeled bike engine formula cars, but they're very pricey. You can go on their site and get a cut-away drawing of one of these differentials, which will answer all of your questions. Their differentials are extremely light, and thus grab prices well into the thousands of dollars.
I've been associated with a Formula SAE (Formula Student, in Europe) team, which has afforded me to witness these things first hand. I can tell you its not that difficult to come up with a design, but you'll want a CNC or at least a lathe and mill to do it well... Look up photographs of Formula SAE/Formula Student team's cars and you'll see tons of examples.
