just use a well lubricated bolt some washers and a nut,

 

and a couple of spanners.

It didn't work for me.. maybe I'm doing it wrong, but the rivnut just spun. I cheated and put it (10-32 unf) on a metric mandrel hand tight and use the normal tool. This worked and the thread was fine for the unc screw afterwards.

you hold the bolt and tighten the nut to pull the bolt up to compress the rivnut.

 

which side of the nut do the washers go?

between nut and rivnut to stop the rivnut moving at the start. hence the well lubed comment. some times two washers help.
