Anyone know which way round the A+ idler gear is meant to go?
The shaft length is equal each side unlike the earlier idler, but there are holes in the ends of the shaft of a dissimilar size.

I think you are over thinking things. it does not matter with these.
the A ones would damage the transfer case if fitted the wrong way.
I half expect to be corrected on this but....

The 'A' type all synchro boxes had off-set shaft lengths and they would only go one way, but the 3 Synch and A+ style will go either way.

I thought it probably didn't matter but am trying to put it back in the same way it came out. From what I can tell from the usage markings it looks like the end with the slightly larger hole went into the gearbox.
It's done 50,000 miles so maybe I should turn it around the other way to spread the wear on the teeth? Or should I leave it so it engages with the old input gear in the way it's used to?

I hear you and used to fret over this myself, but I've come to accept that the teeth don't wear much on these and the shafts usually only go when there's serious misalignment between the Gearbox and Transfer Housing.

I would make every attempt to return it to its original position based on the wear pattern. Mixing and (un)matching gears is never a good idea in my opinion, but I have gotten away with it more times than I care to admit from gearboxes to differentials.
