Sounds like something stuck in the brake disc. Generally failed CV joints click loudly when you drive around tight corners.

Your wheel bearing is dead, the CV joint shaft may also be worn and unusable or the hub itself may be worn. The noise may be the bearing race slipping. The nuts don't just come loose, something must have allowed it to slacken off and you need to strip this hub to find out what. Was the split pin still in place? Taper roller bearings cannot be taken up when they start to wear in that way, once worn they are worn and must be replaced.

I did once have a hub nut that came loose on it's own, i think a previous mechanic had forced a metric nut over an imperial thread (or vice-versa) and completely destroyed it to the point where it would slowly and surely undo itself... As it started to come undone the taper washer would vibrate under braking creating strange creaking/scraping noises like you are describing... Could be worth looking closely at your drive shaft thread/hub-nut to make sure a similar bodge hasn't been done on yours... Also, if it's been running loose the drive flange might be excessively worn on it's inner face - I ended up replacing the flange, cv joint (just because the thread was such a mess), bearings, and hub-nut although mine was kind of an extreme case.

As above - nut coming loose indicates serious problems. Beware -the bearing CAN suddenly seize solid and slew the car - nasty if in the fast lane! I would advise to NOT drive the car until you get this fixed. May now need new CV joint, new taper washer, new nut, possibly new hub carrier - and of course the new bearings it would have needed if you had acted right away.
