When I was taking apart mine I noticed that the base fits onto the door first with two self-tappers - one in the original hole and one more hole drilled into the door itself. Don't think it's the best way to do it though!

Any kind of door-mounted mirror tends to make the door skin crack. If you have the facilities, make up a plate of thicker ali or steel, curved to fit inside the door skin. Make sure the corners are well rounded, or it will tend to deform the skin and leave visible damage on the outside. You can screw directly into that plate, through the skin, or weld nuts to the back.

A good layer of tiger seal or similar will keep it in place, spread the load, and keep water out from between the surfaces.

If it is reasonably large, the plate will reduce vibration of the mirror, which is always a good thing.
