I am sure you could find a suitable tap, or re thread it to a metric thread if you couldnt, 

Or in fact chop up the gearstick and weld on an extension

I made a gearknob 'bar' and my Dad knew someone that could tap it out, Chopping the end off and welding on a bolt would probably be your best bet

Then I got hold of a quickshift (cheap one) but didn't like the ball, so I chopped off the bottom to make a locknut, chopped the sides off, then drilled a hole through it and filed the hole out to a hexagon so that my bar would fit in nicely

All i did to bring it closer to the driver was take a gas torch to it and bend it. Works a treat and is cheaper than a quick shift.
