Brake Pad Fallen Out!

Hi all, got an unusual one here..

 

Heard a grinding coming from my front brake earlier in the week, assumed the pad had worn down to the metal so ordered a new set plus discs as i knew they were on their last legs.

 

When I got the wheel off today I found one of the pads had gone missing, the grinding noise had been the piston on the disc!!

 

What i need to know is why this may have happened, as I can see nothing wrong with my caliper, all split pins in etc.

Really don't want this to happen again and ruin the new disc or worse, cause an accident!

Not an awful lot to see now im afraid as the pads and disc have been replaced

If the split pins that hold the pads in place are still in tact, I can only guess the pads were never in properly or they failed at the split pin hole?

Or the pad broke up an the backplate got twisted and bent enough to go through the the gap between the split pins

I think JackF has the answer as there is no reason the pad should ever come off the split pin unless both pins had broken.
