The brass part should fit into a tube on the bulkhead to locate it, this can break off or an old one could have got rusted in.

Look under the dash and check that the pedal is lined up with the hole in the bulkhead if it's not it will cause the wear your seeing as it drags the cable sideways and wears the cable end. the pedal will bend very easily to realign it with the hole.

This sounds like there is still some brass guide from an old cable in there. My tube fell off after trying to knock out some old brass that had rusted in and I ended up making a bracket to hold the cable in place.

Probably not the issue in this instance but a bad earth can turn the throttle cable into an earth conductor melting it enough in the process to make it stick.
Worth checking the earth strap in the engine bay as the OP did say it was fine for a while with a new cable.

If the old cable end is stuck in there, get a suitably sized self tapper and screw it in, then pull with Mole grips to get it out.
The locating tube on the bulkhead is quite important. But if you don't have welding facilities and it really must be replaced, drill out the old one. Get a suitable sized set screw (fully threaded bolt), nut and some washers. Drill the screw right through big enough to pass the inner cable through. Them drill part way through to enlarge the hole to fit the cable outer loosely. Slightly countersink both ends. Fit through bulkhead with a decent sized washer each side, and a lockwasher and nut. Refit cable and enjoy.
A lathe would make short work of it, but with care you can drill sufficiently close to the bolt centre by hand, preferably in a drill stand. You could probably use an 8mm or 10mm screw from B&Q, as mild steel is ok for this (obligatory warning - but not much else on a car!!!) and easier to drill.

Just seen your post. I will see what I can snap as the engine is in the way but to give you an idea it is just some 2mm steel fashioned into a Z but with right angles and a couple of holes drilled into it.
