Knocking On Rocker Cover

I drove the mini today when it started to shudder and the exhaust fell off. I got it home and when listening to the engine it sounds like a cylinder is off. Also when you feel the rocker cover you can feel the rockers knocking.
Any ideas?
It has been running lean too.

I'd suggest taking the rocker cover off for starters and having a look for tell tale marks in it of contact from the rockers.

 

You could have had a rocker adjusting screw come loose and wound out, or a broken rocker shaft.

 

Or, it maybe something else altogether.

I had a similar problem once, rattling inside the rocker cover, one of the adusting nuts had made itself loose and was knocking around inside,

Alloy rocker cover or standard metal one? Alloy ones are prone to touching especially if 1.5 rockers fitted.
